Community support counselor

Job Summary:We are looking for an organized, compassionate, and highly motivated person to join our team as a Community Support Counselor. In this role, you will provide individuals and families with one-on-one counseling, educational services, and other supports in order to promote healthy living and self-sufficiency. You will work with a diverse population and be responsible for the implementation and evaluation of individualized support plans that are designed to meet the needs of each client.Responsibilities:• Develop individualized support plans for clients, based on their needs and desires.• Provide counseling services to individuals and families, including but not limited to: life skills development, crisis intervention, problem solving, and emotional support.• Facilitate referrals to other service providers, when needed, and ensure that clients have access to the necessary resources.• Monitor client progress and provide feedback to ensure that goals are being met.• Educate clients and families on topics such as communication, parenting, and stress management.• Maintain accurate records and documentation of services provided.• Participate in team meetings and other professional development activities.Qualifications:• Bachelor’s degree in social work, psychology, or a related field.• Experience working with individuals from diverse backgrounds and with various types of disabilities.• Knowledge of community resources and an understanding of the impact of poverty on individuals and families.• Strong interpersonal and organizational skills.• Ability to work both independently and as part of a team.• Excellent oral and written communication skills.• Valid driver’s license and access to reliable transportation.Benefit: • 401(k) matching • Dental Insurance • Health insurance • Health savings account • Life insurance • Paid time off • Vision insurance Powered by Webbtree
